About Kayla Hoffman

I am Kayla Hoffman, a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP). I am dedicated to providing knowledgeable, individualized, and compassionate care that meets the unique mental health needs of each person I work with via telehealth.
I take a functional, collaborative approach to treatment. Together, we’ll develop a personalized care plan that considers the full picture (biological, psychological, and social factors) so the support you receive feels relevant to your life. My care combines thorough evaluation, thoughtful medication management when appropriate, and solution-focused, person-centered support.
I work with children (ages 10+), teens, and adults who feel stuck, overwhelmed, or ready for things to feel better. I meet each person where they are, with clear communication with parents and caregivers when needed, while always respecting the voice and autonomy of the client.

Self-Pay Rates
Cash pay (self-pay) only.
- Initial Visit Evaluation, 1 hour$180
- Follow-up Med Management$85
- Follow-up Med Management with Therapy$215
- FMLA paperwork$300
Sessions extending past the scheduled time are billed at $45 for each additional 15 minutes.
